If your full name, exceeds 30 characters and does not fit in the space provided, shorten your name by using initials. Use a blank space between initials. • Name of the Father/Mother/Guardian:
First select the option given and then write the name clearly. Leave one space between each part of the name and then darken the appropriate box given below the name.

• Date of Birth:
Write your date of birth as recorded in the 10th Pass High School Certificate/Birth Certificate issued by the competent authority in the format provided and then darken the appropriate circle provided below it.
e.g.: If your date of birth is 3rd September 1987, fill as 03/09/1987

• Sex:
Darken the appropriate circle.

• Residence:
Darken the appropriate circle for residence you belong to.

• Annual income of Parents:
Darken the appropriate circle for the total annual income of your parents.

• Category:
Darken the appropriate circle for the category you belong to.

• Test centre Code:
Fill in three test centre options as per the preference and then darken the appropriate circle provided below it. List of Test Centers and Codes are given below:

• Domicile:
Darken the appropriate circle for the domicile you belong to.

• Qualifying Exam:
Candidates who have passed the qualifying examination have to Darken the appropriate circle. Candidates appearing for the qualifying examination in the year 2008 have to Darken the appropriate circle of the examination they have appeared.

• Percentage of Marks & year of Passing:
Candidates who have passed the qualifying Examination should enter the percentage of marks secured in the qualifying examination and the year of Passing. Candidates appearing for the qualifying examination or awaiting result, should leave these columns blank and then darken the appropriate circle provided below it.

• Address for Communication:
Provide your address for communication. All formal communications from CLAT will be sent to you at this address only. Use blank spaces to separate words. Specify the name of your city, state and pin code in the columns provided.
e.g.: If you address for communication is :
# 32, H.F. Colony, 5th Cross, 3rd Main Road SP Nagar, 2nd Phase HAL.
Use the Three lines given to fill up the information appropriately.

• NRI Sponsored:
Darken the circle Yes if you are an NRI sponsored candidate or darken No. NRI sponsored candidates should download the form from the CLAT website, fill it and send it along with the application form to the address given below.

• Occupation:
Darken the appropriate circle for occupation of your parent.

• Preference of the Institutions / Courses based on the relevant Qualifications:
Darken the appropriate circle to indicate the order of preferences to take admission at different institutions and courses.

Procedure for sending the Filled Physical Application Form:
  • Fill all the fields in the Application form
  • Paste (do not staple) recent Passport size (3cm X 4cm) Color photograph.
  • Kindly do the signatures where ever asked.
  • With the application enclose the required copies of certificates
    1. Attested copies All Educational Qualification certificates with mark sheets.
    2. Caste Certificate (if applicable)
    3. NRI Sponsorship certificate (if applicable)
    4. Persons with Disabilities (if applicable)
  • Application with all the enclosures should be sent through Registered/Speed Post/Courier, so as to reach on or before 10th April, 2008, by 5.00 p.m. to the address mentioned below:
